Official Contest Rules
Participating Locations:
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1) HOW TO ENTER
a) The Women’s Workout World “Kenny Chesney” Contest (the “Contest”) will begin on Thursday, May 1st, 2008 at approximately 9 a.m. Central Time (“CT”) and end on Friday, June 6th, 2008 at noon (the “Contest Period”).
b) To participate in the Contest, listeners must stop by a participating Women’s Workout World (a complete list of all participating locations, including the address, city, state and zip code of each is posted above) during the Contest Period during normal business hours to obtain an official entry form while supplies last, with your first and last name, complete address include city, state, and zip code, telephone number and date of birth. Then mail the completed form in a First Class stamped envelope to: US99.5/ Woman’s Workout World “Kenny Chesney” Giveaway, Two Prudential Plaza, Suite 1000, Chicago, IL, 60601.
c) WUSN/US99.5 (the “Station”) is not responsible for lost, stolen, mangled, misdelivered, postage due, illegible, incomplete or late entries.
d) All entries must be received no later than Friday June 6th, 2008 at noon p.m. C.T.
e) Only one entry per person per envelope. No photocopies or mechanically reproduced entries will be accepted. All entries become the exclusive property of the Sponsors and will not be returned.
2) ELIGIBILITY RESTRICTIONS
a) The Contest is open to all US99.5 listeners residing in the Chicagoland Metropolitan Area and Northwest Indiana who are 18 years of age or older at the time of entry. Employees of WUSN, its licensee, its corporate parents, subsidiaries, and affiliates, its advertising agencies, participating sponsors/promotional partners, other radio stations in the Chicago metropolitan area and the members of their immediate families and/or households are ineligible to participate or win. Subject to all applicable federal, state, and local laws and regulations.
b) Only one winner per household is permitted in any station Contest. Listeners are eligible to win a Station Contest only once every sixty (60) days and a prize valued over $600 only once every ninety (90) days. Only one winner per household is permitted in any Contest.
c) Winner must provide government issued photo-ID and sign and return any required Affidavit of Eligibility and/or Publicity/Liability Release.
3) PRIZES
a) Ten (10) prizes will be awarded. Each prize is two (2) tickets to the Kenny Chesney Concert on June 21st, 2008 at Time TBD at Soldier Field in Chicago, IL. Approximate Retail Value per prize: = $133.00
b) All prizes or prize certificates may be picked up at the office of the station at the address below. The winners will forfeit any prize or prize certificate not claimed within three (3) days of winning. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The station is not responsible for the safe arrival of a prize or prize certificate.
c) There is no substitution, transfer or cash equivalent for prizes, except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the items listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
4) SELECTION OF WINNER
a) Decisions of station management with respect to the Contest are final.
b) Ten (10) winners will be chosen on Friday, June 6th, 2008 at approximately 1 p.m. CT in a random drawing of all the eligible entries received.
c) Odds of winning depend upon the number of eligible entries received.
d) Winner(s) need not listen or be present to win.
e) The US99.5 Promotions Department will notify winners by phone on Friday, June 6th, 2008 at 1p.m. CT. An alternate winner will be chosen in a random drawing of the remaining eligible entries received if a winner cannot be contacted within three (3) days of winning the prize. Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form by 5 p.m. on June 9th, 2008, or prize will be forfeited and an alternate winner will not be chosen.
5) CONDITIONS
a) Payments of all federal, state and local taxes are solely the responsibility of the winners. Winners may be required to sign an IRS Form W-9 or the equivalent.
b) By participating in the Contest, the winners and their guests agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this Contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c) Prior to awarding any prize or prize certificate, the Station in its sole discretion may require Contest winners and their guests to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the Contest.
d) The Station reserves the right to make changes in the rules of the Contest, including the substitution of a prize or equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event is delayed, rescheduled, postponed, cancelled, or has a change of venue, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
e) Failure to comply with the Contest rules may result in a Contestant’s disqualification solely at the discretion of the Station.
f) The Station is not responsible for typographical or other errors in the printing, the offering or the administration of the Contest, or in the announcement of a prize.
g) Copies of the written Contest rules and a list of winners (when complete) are available during regular business hours at the main studio of WUSN, Two Prudential Plaza, Suite 1000, Chicago, Il 60601 or by logging on to http://www.wusn.com/Contests.shtml |
