a)     The US99.5/ Miller High Life Ultimate Backyard Makeover contest (the “Contest”) is a promotion that will take place at several different locations by the US99.5 Rewards Team on Saturday, May 3^rd 2008 at McGaffer’s, 7737 W. Roosevelt Rd., Forest Park, IL from 9:00pm to 11:00pm, Saturday, May 10^th at Sarah’s Place, 3601 Channahon Rd, Joliet, IL from 9:00pm to 11:00pm, Saturday, May 17^th Village Inn, 8050 Lincoln Ave, Skokie, IL from 9:00pm to 11:00pm, and Saturday, May 24^th from TBD. To participate in the Contest, listeners must stop by the specific locations during the dates and times listed above, to obtain and fill out an official entry form, available on location while supplies last, and drop it in the designated drop box by the end of the event. Entries must include first and last name, complete address, including zip code, daytime telephone number and date of birth, in order to be eligible for a chance to win. All entries must be received by 10:55pm at each appearance. You must be present to win. The US99.5 Rewards Team at each of the four (4) locations listed above to receive a $100 gift card to Home Depot will randomly draw One (1) winner. The Grand Prize winner will be randomly drawn on Monday, May 26, 2008 at approximately 9:00am from all eligible entries from the four (4) on-site qualifiers.

b)     Oneentry per person, per household please. No photocopies or mechanically reproduced entries will be accepted.

c)      US99.5 and the participating sponsors of this Contest (hereinafter collectively referred to as the “Sponsors”) assume no responsibility whatsoever for any lost, misdirected, illegible, stolen, incomplete, mutilated, or late entries. All entries become the exclusive property of WUSN and will not be returned.

 

a)     The contest is open to all US99.5 listeners who reside in the Chicagoland Area and Northwest Indiana, and who are 21 years of age or older and homeowners at the time of entry. Employees of Miller Brewing Company, Alsip Home and Nursery, the station WUSN(FM), its licensee, its corporate parents, subsidiaries and affiliates, its advertising agencies, participating sponsors/promotional partners, and other radio stations in the Chicagoland Metropolitan Area and immediate families of such persons and those living in the households of same are not eligible to participate or win. 

b)     In order to be eligible to accept the grand prize, the grand prize winner must be the owner of the property in which the prize package will be delivered and installed. Winner must display proof of ownership as required by the Station.

c)      All contestants must be homeowners in order to be eligible, and must be able to provide proof of valid home insurance. Winners are also required to obtain any necessary permits, etc. in order to put on this backyard party.

d)     Listeners are eligible to win a WUSN contest only once every sixty (60) days and a prize valued at $600 or more only once every ninety (90) days. Only one winner per household is permitted in any station contest.

e)     Entry into the Contest constitutes your agreement to be bound by these Official Contest Rules.

f)        The Grand Prize winnermust live with in a 50 mile radius from our stations location (180 N Stetson, Chicago, IL 60601).

 

a)     One (1) winner at each location will receive one (1) $100 gift card to Home Depot. ARV per prize: $100.00

b)     One (1) grand prizewinner will receive a $5,000 gift card from Alsip Home and Nursery and a backyard BBQ hosted by a US99.5 on-air personality and featuring a performance by (a US99.5 Recording Artist) held from TBD on TBD.  US99.5 and Miller Brewing Company will provide food and beverage for up to 50 people.  All event participants must sign an event release form in order to participate. Grand Prize ARV: $7,000.00

 

US99.5 Recording Artist Performance- The Station will coordinate and assume all costs associated with the performance, including installing phone lines and engineering costs, as needed. The Station will also provide the musical entertainment. Miller Brewing Company will be providing food or beverages for up to 50 people. The winner must work directly with the Station’s promotions and engineering departments to set-up technical equipment for the performance. The homeowner must provide at least a 25x25 foot area for the performance on outdoor property that they themselves own. All contestants must be homeowners in order to be eligible, and must be able to provide proof of valid home insurance. Winners are also required to obtain any necessary permits, etc. in order to put on this backyard party. In the event of inclement weather conditions are deemed safe by WUSN for such a performance. We have the right to change everything.                                                              

 

$5,000 grand Prize will be given in the form of s gift card to Alsip Home and Nursery and can be used at the winner’s discretion.  Winner is not permitted to make substitutions to any prize component. There will be no refunds or exchanges for prize components or materials that are not used.If the winner chooses to forfeit the backyard performance, the entire prize is forfeited. The Alsip Home and Nursery gift Card has no expiration date. No substitutions by winner are allowed. Prize Provider may substitute equal value materials or prize components at its discretion. The station is not responsible for any property damage or personal injury in relation to this prize. 

 

c)      All prize certificates may be picked up at the office of the station at the address below. The winner will forfeit any prize certificate not claimed within forty-five (45) days of winning. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The station is not responsible for the safe arrival of a prize or prize certificate.

d)     There is no substitution, transfer or cash equivalent for prizes, except that Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.

 

a)     Decisions of station management with respect to the contest are final.

b)     At approximately 10:55pm during the station appearance (see above for locations, dates and times), the US99.5 Rewards Team will randomly select one (1) winner from all of the eligible entries received up to that point to receive one (1) $100 Home Depot gift card. Prizes will be awarded upon confirmation of contest eligibility.

c)      One (1) Grand Prize Winner will be chosen at random by the US99.5 promotions Department from among all $100 gift card winners on Monday, May 26, 2008. The Promotions Department will contact the winner by telephone. If winner cannot be reached within three (3) days or cannot accept prize for any reason, he/she will forfeit prize and an alternate grand prizewinner will be selected in a random drawing of the remaining winners of the $100 gift card and on-line entries.

d)     Winners need not listen to win but must be present to win (grand prize winner does not need to be present to win). If winner is not present or does not claim prize within three (3) notification attempts after the name has been called, another name will be drawn until an eligible winner is found. 

e)     Odds of winning depend upon the number of eligible entries received.   

 

a)     Entry into the Contest and acceptance of a prize constitutes permission, unless prohibited by law, to use the winner’s name and guest’s name, voice, or likeness in any advertising or broadcasting material relating to this contest without additional financial or other compensation.   

b)     Payments of all federal, state, and local taxes are solely the responsibility of the winners. Winners may be required to sign a W-9 IRS Form or the equivalent.

c)      By entering into the Contest and accepting a prize, the winners (and their guest, if any) agree that the Sponsors, their parents, partners, subsidiaries, and affiliated companies, their respective agents, employees, directors, officers, shareholders and legal advisors and any other companies participating in the design, administration, promotion, advertisement or fulfillment of the Contest (hereinafter collectively referred to as the “Contest Entities”), shall not be liable for personal injuries, death, damages, expenses or costs or losses of any kind resulting from participation or inability to participate in the Contest or acceptance of or use or inability to use the prize(s) or parts thereof including, without limitation, claims, suits, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light (whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. The winners release the Contest Entities from any and all liability and responsibility with respect to any prize (including any property loss, damage, personal injury or death).

d)     Prior to awarding any prize or prize certificate, WUSN, in its sole discretion, may require contest winners (and their companion(s), if any) to sign a liability release agreeing to hold the Contest Entities, WUSN, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents, and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. 

e)     WUSN reserves the right to make changes in the rules of the Contest, which will become effective upon announcement. If due to circumstances beyond the control of the station, any competition or prize-related event is delayed, rescheduled, postponed, cancelled or has a change of venue, the station reserves the right, but not the obligation, to cancel or modify the contest and shall not be required to award a substitute prize.

f)        US99.5 is not responsible for air traffic or other travel or event delays, cancellations, postponements, change of venue or rescheduling due to circumstances beyond its control. In such event, US99.5 reserves the right, but not the obligation, to cancel or modify the contest and shall not be required to award a substitute prize.

g)     Failure to comply with the Official Contest Rules may result in a contestant’s disqualification solely at the discretion of US99.5.

h)      WUSN is not responsible for typographical or other errors in the printing, the offering, or the administration of the contest, or in the announcement of the prizes.
